Lamb’s Fold case managers help the residents identify their assets and short and long-term goals.
Case managers help residents along the way, in ways such as: school enrollment; obtaining appropriate child care; learning more about parenting; ensuring access to proper medical care, helping apply for disability benefits and other entitlements, etc. Our case managers work with the resident to set specific time frames for completion of goals and offer encouragement and assistance to achieve them.
Case Managers monitor progress toward goals and soon begin the active phase of transition to self-sufficiency. Employment is a big focus of our program and Lamb’s Fold works with area employers to assist residents in job search and job coaching. Residents have the opportunity to work with their case manager or other staff to develop/update a resume and utilize the internet for job searches.
Education is also an area of strong focus, and case managers encourage every resident to get a high school diploma/GED and move on to higher education as appropriate.
Case managers travel to each woman’s home and provide services to her there; residents also come to the Lamb’s Fold offices for classes, workshops, resumes, etc.
